Special Note: Changes To Your Personal Details
If you would like to change any of your personal details such as your name, marital status or your nominees we will require notification in writing. For changes of name or marital status your letter should be accompanied by either your original certificates (which will be returned to you recorded delivery) or certified copies of the originals. Please note this office will only accept copies that have been certified by the relevant contact at your employer, a doctor, a solicitor or a religious minister.

Special Note: Change To Your Address
If you are notifying this office of a change of address by email or letter, you should include details of your Member Number, date of birth, National Insurance number and previous address.
